Q: Who can live at My Forever Home?

Residents living at My Forever Home are adults 18 or over with a diagnosis of an Intellectual or Developmental Disability interested in affordable, community living using the Supported Living Model.
Q: What is Supported Living?

Supported Living is an NC Medicaid Innovations Waiver service available to Innovations Waiver recipients. Supported Living services are intended to provide people with any level of disability the opportunity to live in their own home while receiving the services and support they need to live their lives.
Q: When will My Forever Home be completed?

The first phase of My Forever Home, the first 4 homes, will be built by August 1st. The second phase, the next 3 homes, will be built by November 1st and the third phase, the last 3 homes, will be built by February 1st.
Q: Where is My Forever Home Located?

My Forever Home is located in the Finley Landing Subdivision at 2227 US Highway 70 Business West in Smithfield, North Carolina.
Q: Why are you building My Forever Home?

My Forever Home Offers inclusive affordable housing where folks with intellectual and developmental disabilities can live, learn and grow with the support they need to thrive.
Q: How much will it cost to live at My Forever Home? How will people pay?

My Forever Home offers affordable rentals at half of the current rental rate. Residents can use SSI, SSDI, or private pay to pay rent and the NC Innovations Waiver or private pay to pay for the support agency they choose.
Q: Are the homes handicap accessible?

My Forever Homes will be one level, single family, 2 bedroom homes with each resident having their own bedroom. They are zero entry homes (no stairs to enter) offering an open concept floor plan with 2 houses having handicap accessible bathrooms.
Q: Will you offer a parent group?

My Forever Home will offer a parent committee for those interested in getting involved in the MFH community by volunteering and offering their time and talents to support the residents. We will also meet to talk about topics centered around our adult children with disabilities such as special needs trusts, independent living skills or how to manage money.
Q: How will residents pay for food and other necessities?​

Residents can use SSI, Mom's Meals, SNAP benefits and/or private pay to pay for groceries, cleaning supplies and other necessities.
